We’re thrilled that you’re considering Bajaj & Co. Endocrinology for your care! Here’s how to get started as a new patient:

 

Referral Process:

If you have never been seen by one of our physicians, please have your primary care provider or referring physician send us a referral. This allows us to review your medical history and ensure you are scheduled with the right provider for your needs.

 

Important Note: This physician-to-physician referral is required for all new patients, regardless of insurance coverage. It is not the same as an insurance referral.

• If you have been seen by a Bajaj & Co. Endocrinology physician before, you will continue with the same provider, as we do not transfer patients between physicians within our practice.

Preparing for Your Appointment:

We value your time and strive to keep our schedule running smoothly. To avoid delays, please review our appointment guidelines:

• New Patient Forms – Download and complete your forms in advance using the link below. Bring them with you to your appointment for a faster check-in experience.

• Timely Arrival – If you prefer to complete your paperwork in our office, arrive at least 30 minutes before your scheduled time to allow for processing.

• Late Arrivals – Patients who arrive late or without completed paperwork may be asked to reschedule to maintain the flow of appointments.
