• If it has been more than three years since your last visit, you will be considered a new patient and will need to complete updated new patient paperwork at your next appointment. However, you will still be scheduled with your previous physician.
   

• We offer a 15-minute grace period for follow-up appointments. If you arrive 15 minutes or later beyond your scheduled time, you may need to reschedule.
